Your Private Health Information
As a RMT, I am required to keep your health information confidential and secure.  Your file will be locked in a filing cabinet that only I have access to.  I cannot share your information with anyone without your written consent except in the following situation:

1. Peer Assessment from College of Massage Therapists of Ontario

2. Complying with a court order or other legal requirement

3. When certain type of abuse is suspected

If I feel you can benefit from another therapist or health care professional, I will make a referral with your written consent to share your information.

Cancellation & Late Policy
To respect my time and other clients time, please give me at least 24 hours of notice if you want to cancel or reschedule.  I travel for most of my appointments so it will take more time to prepare and arrange schedule.  

For change in appointment date and time within 24 hours, you will be billed for $35.00.  

For NO SHOW (appointment is missed), you will be billed for $60.00.
